The Perth County Ontario Provincial Police (OPP) is continuing to investigate a serious two-vehicle, head-on collision that occurred on Perth Road 113 (Embro Road), just south of Stratford, on Sunday evening, July 20.

The crash involved a southbound Pontiac Grand Am carrying three Municipality of West Perth residents, and a full-sized Chevrolet van carrying a family of seven from the Municipality of West Perth.

According to the OPP, investigation revealed that the Grand Am crossed over into the northbound lane of the Embro Road and collided head-on with a northbound van. The impact resulted in injuries to the occupants of both vehicles.

The Grand Am was carrying two adults and a child as occupants, while the van was carrying two adults and five children.

The child passenger in the Grand Am was airlifted to a London hospital with major injuries. The adult passenger of the van was also airlifted to a London hospital with major injuries. The remaining eight injured persons were transported by land ambulance to a local hospital.

The crash occurred around 5:10 p.m., north of Line 20. The roadway remained closed for clean-up and investigation until around midnight on Sunday.

The driver of the Grand Am, Tammy-Lynn Dingman, age 42, faces the following Criminal Code of Canada offences: “breath sample: not provided upon demand,” “impaired driving causing bodily harm,” and “dangerous operation causing bodily harm.” She is scheduled to appear at the Ontario Court of Justice in Stratford on Sept. 8. In addition, she was issued a 90-day driver’s licence suspension.
